Dietitians are primary care practitioners and you can make an appointment directly without a referral. If you have been diagnosed with a chronic medical condition and require complex care you may be eligible for Chronic Disease Management (CDM) assistance. In this case, the GP completes a special referral form to refer you to a dietitian for treatment.
1 - Current Care Plan Referral by your GP,
2 - Centerlink Card (Pension, Health or Concession)
3 - Medicare card.
A Care Plan is valid for the stated number of services (sessions) decided by your GP.
If all services are not used during the calendar year in which the patient was referred, the unused services can be used in the next calendar year.
However, those services will be counted as part of the five rebates for allied health services available to the patient during that calendar year.
Please note that our Dietitian and Diabetes Educator at Allied Osteopathy may apply a fee (with rebate) only on the first consultation of the care plan. Please call for more information.
